The t:slim X2 Insulin Pump is a tubed pump. The technology of this device was designed such that patients with tubed insulin pumps could control their devices remotely. Their “Control-IQ” technology claimed to provide “freedom” while using a smartphone application to allow for more flexibility for patients. Tandem advertises the t:slim X2 Insulin Pump as the number one, automated insulin delivery system; however the t:slim X2 Insulin Pumps have been responsible for the severe injury of hundreds of their users. Due to their Apple iOS mobile applications crash in early 2024, the software was recalled as the application was prone to crash and automatically relaunch. This resulted in inconsistent insulin administration and blood sugar measurements.
As of April 15, 2024, over 200 injuries were reported as a result of this software failure. Essentially, the device was not communicating with the application without the knowledge of the patients. The crash and reboot reports from this application led to faster battery drain, pump shutdown resulting in under-delivery of insulin, which can and have resulted in life threatening situations for diabetic patients. Fortunately, no deaths have been reported as a result of this software failure; however, the number of injury reports are on the rise and the conditions that people are suffering can certainly be fatal.
On March 18, 2024, the Food and Drug Administration identified the recalled software as a Class I recall, the most serious class of recalls. The FDA acknowledged that the devices are a risk for causing serious injury or death.
